Your personalised supportYou (service user) look at the Opportunities for Life and decide in which area(s) you would like to develop. You may find it helpful to look at the details of the modules on the Stepping Forward Self Assessment to help you decide. Assess yourself on Step 1 of the chosen module(s). Tick the items you know or can do and put an SFagainst areas that you want to work on. Items with SF against them should be transferred to your support plan. When you complete work on a Step, you can put a date of completion on the My Achievements sheet. This will allow you to see your progress over time.
