Your personalised support

You (service user) look at the Opportunities for Life and decide in which area(s) you would like to develop. You may find it helpful to look at the details of the modules on the Stepping Forward Self Assessment to help you decide.

Assess yourself on Step 1 of the chosen module(s).  Tick the items you know or can do and put an SFagainst areas that you want to work on.

Items with SF against them should be transferred to your support plan.

When you complete work on a Step, you can put a date of completion on the My Achievements sheet. This will allow you to see your progress over time.

Stepping Forward
Includes prompt questions with space for service user responses providing easy outcomes evidencing.
                               
Tenancy Ready  
Tenancy Ready status will be achieved when the following modules are completed: Stay Safe, Money Matters (Maximising my Income and Managing my Money), Move On and some elements of Be Healthy. These can be found labelled TR. See labelling on My Achievements to monitor progress on this journey.
                       
Support Plan
You can use this, or your own Project´s support plan to decide on progress targets and support.

My Achievements 
This is where you mark your achievements on your work.

Certificates      Use for praising completed steps.
